Medical Records

As our patient, you have full access to your medical records. We have put procedures in place to keep your records safe and confidential as required by law, while at the same time allowing our health care providers access to your records to provide you with quality patient care. We release medical records according to the Health Insurance Portability and Accountability Act 1996 (HIPAA), New York State, and Federal Laws.

As our patient, you have the right to:

  • Access or inspect your medical records
  • Obtain a copy of your medical records as long as the information is retained
  • Inspect and get a copy of our Notice of Privacy Practices
  • Request a change to your medical records
  • Request a list of who we disclosed your records to
  • Request restrictions and limitations on disclosures of your health information
  • Receive notification of any breach of unsecured health information
  • Voice your concern or complaint about our privacy practices

If you wish to request an amendment or limitation on your medical records, please visit one of our Health Information Management (formerly the Medical Records) Departments at the Mount Vernon Neighborhood Health Center, Yonkers Community Health Center, and Greenburgh Health Center locations.

Interested in viewing your medical record only? Existing patients can use our free online Patient Portal to view their up-to-date records. Lab results and immunization history can also be viewed and printed. Sign up to get your patient portal account.

How to get a copy of your medical records

Existing and past patients of Mount Vernon Neighborhood Health Center, Inc. (MVNHC) can access their medical records:

  • In-person at a Health Information Management Department
  • By email
  • By fax
  • By mail

In-person at our Health Information Management Department

If you prefer to get your records in person, you may visit one of our three Health Information Management (HIM) Departments during regular business hours. HIM Departments are at the Mount Vernon Neighborhood Health Center, Yonkers Community Health Center, and Greenburgh Health Center locations.

Current picture identification, with signature, must be presented.  You will also be required to sign our HIPAA Release Authorization Form (in English) or HIPAA Release Authorization Form (in Spanish) to receive your records.

Requesting medical records for an outside provider? Complete Item #6 on our HIPAA Release Authorization Form to release information to an outside provider. Medical records can be faxed, mailed, or emailed. Your medical record copies are processed within 5-10 business days after submitted requests. Medical record requests for a continuum of care are sent to other providers without a fee.

Requesting medical records for a minor (under the age of 18 years)? Parents and legal guardians have the right to request dependent children’s records before 18 years by completing a signed HIPAA Release Authorization Form (in English) or HIPAA Release Authorization Form (in Spanish) with proper identification. 

Please note: Certified Birth Certificates are NOT available from any of our HIM Department locations. These records must be ordered from the Department of Vital Statistics in the state where the birth was registered. 


By email

You can email us your request for medical records. Please download, complete, and attach a completed HIPAA Release Authorization Form (in English) or HIPAA Release Authorization Form (in Spanish) in the body of your email and include “Request for Medical Records” as your subject line. (By including a subject line, it will help to expedite your request.) Please indicate in your email how you would like your records returned to you. You may request that we provide your records to an outside entity by filling in Item #6 of the form. We attend to email requests on a daily basis.

By fax

You may also fax in a request for your medical records using our HIPAA Release Authorization Form (in English) or HIPAA Release Authorization Form (in Spanish). Please indicate how you would like your records returned to you. If you are acting on behalf of an outside medical provider, please indicate how they would like to receive your medical records.

By mail

If you wish, you may mail us a request for your medical records. Please complete the HIPAA Release Authorization Form (in English) or HIPAA Release Authorization Form (in Spanish) and indicate how you would like your records returned to you. You may also make a request on behalf of an outside provider.
