What is Patient-Centered, Coordinated Care?
Your medical care may involve several health professionals and services. Patient-centered, coordinated care involves your health care team working together with you to achieve your health goals.
Your primary care physician heads your health care team. He/she coordinates your care across our centers and with various departments and professionals.
At the time of registration, all our patients select a primary care physician to oversee their care. Your primary care physician is responsible for creating a personalized care plan with you and explaining the plan to you in a manner that you can understand. Your care might involve other medical professionals and support services, which your primary care physician will then oversee and coordinate. If you need care outside our centers or need to be admitted to a hospital, your primary care physician will be actively involved in monitoring your progress.
