Guiding you to the care and services you need.

Navigating through any health care organization can be challenging. Our Patient Advocates are here to help guide you through the services and programs we offer to get the best health care possible for you and your family. They can also help you interact with your health care providers and listen to your care concerns.

Helping a Patient

Our Patient Advocate can help you:

  • Communicate your needs to your health care provider to ensure you receive consistent care over time and to encourage your active participation in your plan of care for a better health outcome
  • Provide you or a family member with an on-site language interpreter
  • Connect you with a language line service for all languages including American Sign Language
  • Schedule care appointments or arrange in-house courtesy patient transportation to your medical appointments
  • Schedule appointments for specialty services in-house or link to outside specialty services as referred by your primary care provider
  • Enroll you in programs that we offer like food programs or health and wellness classes
  • Find out about and connect you to our available in-house programs and resources such as the WIC Program, the PICHC Program, and discounted prescription drugs (the 340B Program)
  • With the application process to obtain Medicaid medical transportation
  • Get you in touch with available resources in your community, like housing or aid programs
  • Communicate your concerns and the way you want your treatment to benefit you, including documenting any complaints or issues you might have and informing you of our patient grievance procedures
  • With referrals for enabling services such as Home Health Attendant Services (HHA), Durable Medical Equipment (DMEs) such as walkers, wheelchairs, hospital beds, incontinence supplies, and other medical equipment needed to your activities of daily living.
  • Learn about and provide counseling support on preventive health, and self-management of diseases, and assist you with adherence to medical treatment and follow-up care given by your providers

How do I get in contact with a Patient Advocate?

Patient advocates are available at our Mount Vernon, Greenburgh, and Yonkers locations. They are always in the building during business hours – helping patients or in their office. Just ask at the front desk or a health care provider to direct you to them.
